Speech Language Pathologist/Rehab Manager Position in a Home Health Care Setting in Santa Barbara/Santa Ynez Valley, CA
1. Monitors nursing care and patient outcomes for adherence to care plan, meeting patient needs, producing desired outcomes, and interdisciplinary care.
2. Assists Director of Home Health in the planning, implementation and evaluation of inservice and continuing education programs. Assists in formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives both short and long term.
3. Works with the Director of Home Health to maintain and evaluate agency/ program budget. Seeks opportunities to reduce operating costs.
4. Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
5. Initiate conference with program director regarding any questions about individual eligibility for services.
6. Attends case conference meetings with organization personnel to facilitate coordination of care.
7. Conducts monthly record reviews and communicates findings and recommendations to Director of Home Health and appropriate organization personnel.
8. Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Hires, mentors, evaluate, counsels, and disciplines employees.
9. Assists in the orientation of new nursing personnel. Ensures initial and annual competencies are completed.
10. Responsible to complete employee performance evaluations on probationary and annual basis through provision of supervised Home Health visit. Clinical evaluation for performance includes observation of competencies, clinical documentation and compliance with regulatory and accreditation standards.
11. Completes accurate, timely employee performance evaluations on probationary and annual basis. Annual evaluation includes documentation to position and regulatory and accreditation standards.
12. Counsels and/or disciplines employees in a timely, confidential manner. Encourages employee problem solving, conflict resolution, and improved performance. Seeks appropriate guidance and direction for difficult disciplinary situations.
13. Develops and implements a preceptor and/or mentoring environment for hospice nursing staff. Preceptors/mentors are selected on identified criteria and evaluated during and at the completion of the mentoring process for effectiveness and suggestions for improvement.
14. Maintains procedures, protocols, processes and communication that facilitates team collaboration, effectiveness of therapy practice, and adherence to standards of practice. Researches “best practices” to assure most current therapy practice is promoted for optimum patient outcomes.
15. In conjunction with hospice management team, develops implements and evaluates processes for staff development, growth, professional goal setting, and pursuit of excellence in delivery of home health care. Assists in the planning, implementation and evaluation of in-service and con¬tinuing education programs. Assesses needs of therapy staff and assists in the development of program(s) to meet needs.
16. Complies with accepted professional standards and principles.
17. Participates in public relation and community activities that promote the Visiting Nurse & Hospice Care’s role as an effective member of the health care delivery system.
18. Has working knowledge of state, federal, local and accreditation (CHAP) regulations for the delivery of home health services.
19. Able to prioritize and facilitate interventions that effectively solve or prevent problems related to systems deliver and day to day care.
20. Demonstrates effective planning by assessing there is sufficient staff to perform the duties and responsibilities of the program.
21. Participates in on-call duties as defined by the on-call policy.
22. Assesses health needs of community and assists in the development of plans/ programs to meet needs. Participates in public relations and community activities that promote the organization’s role as an effective member of the home health delivery system. Promotes customer service orientation to all home health personnel.
23. Participates actively in quality management program.
24. Performs other duties and activities as delegated by the Director of Home Health.
Compensation: $65K+/year.

Job Requirements
Must have a degree as a Speech Language Pathologist.
Must have a license to work in the state of California.
Two years management or supervisory experience in health care setting, preferably home care.
